Clinical Liaison- CM/ RN

Position Summary

• Under general administrative direction, coordinates referrals from physicians, case management, and health care facilities for patients.
• Responds to and performs pre-admission screening for patients referred to the rehab program.
• Provides a significant level of patient education related to illness and rehabilitation recovery process.
• Responsible for guiding patients with rehabilitation DRGs through the discharge rehabilitation care continuum.
• Responsible for the development and implementation of referral program as a means to promote the feature and benefits of the rehab program to patients, families and the community.
• Collaborates among all disciplines to ensure that patient’s receive care that consistently follows evidenced based standards.
• Completes established competencies for the position within designated introductory period.
• Other related duties as assigned
