The Licensed Practical Nurse Care Coordinator will review patient records to verify information accuracy and ensure documents reflect the patient's conditions and treatment requirements. Responsibilities also include submitting Review Visit Logs, adhering to operational guidelines, responding to various correspondences, and attending scheduled meetings.
Candidates must possess an LPN certificate or degree and the ability to work full-time. Essential soft skills include being empathetic, an active listener, detail-oriented, and possessing strong time management, communication, and interpersonal skills.
