The Social Worker will be responsible for discharge planning and coordination, assessing patients' social care needs to ensure safe and timely discharge from the hospital. They will also provide safeguarding and advocacy, emotional and practical support to patients and families, and arrange necessary support services post-discharge.
Candidates must have Social Work England registration, be eligible to work in the UK, and hold a full UK driving licence. A degree level or equivalent in Social Work and post-qualified experience are also required.
Liquid Personnel is currently recruiting for an Adult Social Worker to join its client's Hospital Team based in Liverpool.
