The coordinator will engage individuals empathetically, screen for co-occurring disorders, and assess needs to develop strength-based, stage-wise, person-centered treatment plans. Responsibilities also include completing necessary documentation, making referrals for community-based services, and advocating for client support needs.
Candidates must possess a Bachelor's degree with three years of supervised experience, or a Bachelor's degree with specific Michigan licensure (LLBSW/LBSW) and one year of experience, or a Master's degree with specific Michigan licensure (LLMSW, LMSW, LLPC, LPC, TLLP, or LLP) and one year of specialized experience. All candidates must be a CMHP in accordance with Medicaid Provider Manual Guidelines.
