The Program Coordinator is responsible for developing and monitoring client goal achievement and maintaining individual support plans. They also provide service coordination, medication management, and facilitate communication between the client's care team and external stakeholders.
Candidates must hold an Associate or Bachelor's degree in a related field and possess at least one year of experience as a Program Specialist. A valid Wisconsin driver's license and a clean driving record are required, along with the ability to obtain CPR/First Aid and Medication Administration certifications.
This position is responsible for developing and monitoring goal achievement and other documents as needed, of all clients served with regular contact with the client’s care team.
