The Team Care Coordinator acts as the primary liaison between patients, providers, and the healthcare team to ensure smooth care delivery and guides patients through their healthcare journey. This role supports providers by handling administrative and clinical tasks, allowing for more direct patient care.
Candidates must possess a High School Diploma/GED or equivalent, with preferred medical background or coursework, and strong organizational, typing, and computer skills. Essential requirements include excellent interpersonal skills, the ability to multitask under pressure, maintain confidentiality, and achieve a typing speed of 40+ WPM.
