Job Description
Performs nutrient intake analysis, assesses nutritional needs of patients, develops and implements nutritional care plans for Ryan White patients at UF CARES and within Ryan White Jacksonville network.
Assesses learning needs and readiness of patients and families; provides individualized age-appropriate nutrition information to patients and families, ensuring comprehension and re-educating as needed.
Participates in discharge planning or patient care/case conferences; follows-up on recommendations and care plans with team; coordinates and transitions care from inpatient to outpatient or outpatient to inpatient services; participates and develops pertinent nutrition related programs as needed.
Provides for counseling of patients and/or their support network when special dietary needs indicate that explanations and/or instructions are needed.
Consults and works with community resources, professional organizations and other agencies in implementing care plans.
Documents nutrition information in the electronic medical record. Maintains all records and reports.
Participates as a member of the healthcare team and acts as liaison between patients, families and the clinical team to ensure quality customer service.
Assists with developing, updating, and revising of nutritional educational materials, including annual review of dietary manual. Develops and presents health education and promotion programs, including training workshops, conferences, and community presentations.
Attends and participates in staffing, department and hospital-wide meetings
Expected Salary
$52,000 to $65,000 annually, depending on education and experience.
Minimum Requirements
Bachelor's degree in dietetics, foods and nutrition or food service administration, completion of American Dietetic Association approved dietetic education program. Licenses in Dietitian in accordance with Chapter 468, Florida Statutes.
Preferred Qualifications
Two years clinical experience in a healthcare setting and/or with HIV/AIDS population preferred.
Master's degree in an appropriate area of specialization; or a bachelor's degree in an appropriate area of specialization and two years of appropriate experience
