The Care Manager fosters patient autonomy by providing guidance, education, and assistance as patients navigate the healthcare continuum, developing care plans in collaboration with providers and families. This role involves coordinating care, providing disease-specific education, and ensuring smooth transitions between various healthcare providers and community resources.
Candidates must have completed an accredited Registered Nursing (RN) program and possess an unrestricted RN license by the State of Ohio. A minimum of two years of experience in a healthcare setting is preferred for this role.

As the Care Manager - RN for NOMS Healthcare, you will foster patient autonomy by providing patients with information that enhances their ability to make appropriate health care decisions and/or receive medical care with an enhanced sense of confidence when making healthcare decisions. The Care Manager provides individual guidance, education, and other assistance to beneficiaries as they move through the healthcare continuum developing care plans as a guide. This role involves collaboration with healthcare providers, patients, and their families to facilitate effective and efficient healthcare delivery.
1. Completion of an accredited registered nursing (RN) program 2. Minimum of two years’ experience in health care setting preferred. 3. Unrestricted, Registered Nurse licensure by the State of Ohio

The work environment is well-lit, well-ventilated, and designed for comfort and efficiency. This position requires sitting for extended periods, as well as occasional stooping, bending, or reaching. The ideal candidate must be able to view computer screens for long durations and work effectively in a fast-paced, collaborative office setting. For patient care–related roles, the position may also require standing for long periods and the ability to lift or assist patients, medical equipment, or supplies — generally up to 25–50 pounds, as needed to support safe and effective care.
