The Case Manager III serves as the primary support and case manager for individuals with mental illness and co-occurring disorders, assisting them to live as independently as possible within their chosen community. This role requires proficiency based on additional education and experience compared to a Case Manager II.
Candidates must possess either a Bachelor's degree in Social Services or Human Services plus one year of experience, or a High School/GED equivalent plus four years of direct services experience in Mental Health or community services, with an Associate's degree substituting for two years of experience. A valid driver's license and personal vehicle insurance are mandatory for driving clients to appointments.

Under direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Case Manager III is distinguished from Case Manager II by the additional education, years of experience, and ability to proficiently perform the job functions and meet position standards.

Expected starting wage range is $22.17 - $27.08. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

This cluster of programs provides services to a mix of populations including adults, older adults, transition-aged young adults, individuals with co-occurring disorders, those who are uninsured or underinsured, and persons representing underserved groups in Stanislaus County.
