The Care Manager will conduct thorough client needs assessments and develop tailored support plans in collaboration with clients, families, and agencies. This role involves coordinating community resources, advocating for clients' needs, and monitoring the progress of support plans to ensure quality of life improvements.
Candidates must possess fluency in both English and Spanish, a valid driver's license, and proven experience in non-medical care management or social services. A Bachelor's degree in a related field like Social Work or Psychology is preferred, along with strong organizational and communication skills.
The Care Manager provides Enhanced Care Management services to assigned members. This role focuses on member engagement, care coordination, and documentation in alignment with payer and regulatory requirements.
The Care Manager is responsible for completing required ECM activities accurately, on time, and in accordance with established standards.
Workweek, compensation, and benefits
