The Case Manager is responsible for maintaining the safety and security of residents and the facility, handling timely crisis notifications, and managing the intake process for new residents. Duties also include developing and updating Individual Service Plans, making necessary referrals to external agencies, and maintaining accurate case files.
Candidates must possess a minimum of a High School Diploma or equivalent, with an Associate's degree or higher and supervised counseling experience in substance use treatment being preferred. Essential requirements include professionalism, flexibility, excellent communication, basic computer skills, and the ability to work collaboratively while understanding substance use and mental health relationships.
Our Transitional Support Services Programs (TSS) offer services that bridges the gap between detox and step-down services to residential after-care.
