The Case Manager will educate and motivate patients regarding the prevention and self-management of mental health and substance abuse issues, while formalizing case plans, strategies, and summaries in written form. They will also perform data entry, report clinical measures, assist providers, and coordinate goal-achievement plans for patients not meeting clinical goals.
Candidates must possess a Bachelor's degree in Social Work or an appropriate field, along with current BLS and Mandatory Reporter certifications. A minimum of two years of experience in case management or a related field is required, along with knowledge of community resources and understanding of barriers to medical care.
