The Care Navigator is responsible for meeting with eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. This role also involves communicating eligibility status to senior leadership and relevant departments, and routinely collecting and entering Outpatient eligibility data into Midas for reporting.
Candidates must possess a Bachelor's degree and have one to three years of related work experience. The role is based at a new behavioral health facility in Bethlehem, Pennsylvania, working with seniors, adults, and adolescents.
