The Care Coordinator completes and documents discharge planning assessments, facilitates the development of multidisciplinary discharge plans engaging relevant team members and post-acute providers, and oversees the implementation of these plans. They collaborate to ensure progression of care, appropriate resource utilization, and timely transitions to post-acute services.
Candidates must be a graduate of an accredited school of nursing and hold an RN license. A minimum of two years of acute hospital clinical experience is required, or a Master's degree in Case Management or Nursing can substitute for one year of experience.
