The Medical Social Worker will establish care plans aligned with the physician's treatment plan, provide necessary crisis intervention, and assist caregivers in understanding factors affecting patient medical improvement. Responsibilities also include educating patients and caregivers and making appropriate community referrals for resources like housing or public assistance.
Candidates must possess a Master’s degree from a CSWE-accredited school and must be certified under Indiana code IC25-23.6-4-3 if working in Indiana. A minimum of one year of social work experience in a healthcare setting, current CPR/BLS certification, and current state licensure are also mandatory requirements.
