The Primary Care Physician provides the full scope of services within their field, including diagnosis, treatment, care coordination, and preventive care for patients at the Eskenazi Health Center team. They are expected to uphold the mission and objectives of the Center while complying with all relevant policies, procedures, and regulations.
Candidates must possess current MD licensure in Indiana, current DEA and CSR registration, and be Board Eligible or Certified in their specialty, achieving certification within three years if eligible upon hire. Additional requirements include insurability, current BLS or PALS certification, and the ability to meet Eskenazi Health credentialing standards.
1. Current licensure as an MD to practice in the State of Indiana by the State Board of Medical Examiners.2. Current DEA, CSR Registration3. Board Eligible or Boards Certified in physician specialty. If Board Eligible at the time of hire, must be Board Certified within 3 years of hire date4. Insurability (malpractice)5. Current BLS or PALS6. Must be able to meet and maintain current Eskenazi Health credentialing and privileging requirements.
