This role involves performing daily concurrent medical record review to assess medical necessity and level of care appropriateness, along with post-hospital planning and managing quality and risk management issues. The Case Manager actively assists physicians and staff by facilitating patient care through interaction with hospital departments and community services to assure the continuum of care.
Minimum requirements include two years of nursing experience in an acute hospital setting and a current California RN license. Preferred qualifications include two years of case management experience in an acute hospital within the past year, and a current BLS (CPR) certificate is preferred.
