The Home Health Licensed Practical/Vocational Nurse administers skilled nursing care to patients in their residence according to physician orders and the plan of care, under the supervision of the Registered Nurse Case Manager. Responsibilities include implementing care plans, providing treatments, observing patient conditions, teaching patients and families, and maintaining accurate clinical records.
A certificate from an accredited Practical or Vocational Nursing program approved by the state is required, along with an active and unencumbered Licensed Practical Nurse or Licensed Vocational Nurse license in the state(s) of employment. Two years of experience as an LPN in a clinical setting, preferably in skilled nursing, hospice, home health care, oncology, or emergency care, is preferred.
