The Physician Assistant or Nurse Practitioner provides comprehensive diagnostic and therapeutic patient care in collaboration with an attending physician, functioning as a focal professional for continuity of care within their scope of practice. Responsibilities include performing assessments, managing treatment plans, educating patients and families, and ensuring high standards of ethical and professional conduct.
Candidates must graduate from an accredited Nurse Practitioner or Physician Assistant Program and must obtain necessary Connecticut state licensure and certifications such as NCCPA certification, ACLS/BLS, and potentially DEA/PALS/NRP depending on the specific role. Clinical working experience within the specialty is preferred but not required for initial consideration.

The Physician Assistant or Nurse Practitioner is a licensed professional provider who functions in collaboration with an attending physician in a variety of practice settings at YNHHS. Provides exceptional direct diagnostic and therapeutic patient care to the highest standards of the organization as part of a multidisciplinary team. Cultivates a patient centric environment which focuses on the whole individual inclusive of physical, psychosocial, spiritual and functional needs of the patient, family, and significant others. Serves as the focal professional for the continuity of care in patient care within scope of practice standards, as well as specialty specific standards to address and resolve problems that may arise.

Join a collaborative, patient-centered hepatology team in a dynamic outpatient environment. Provide care for patients with acute and chronic liver diseases across multiple clinic locations. Work closely with a multidisciplinary team including physicians, nurses, and specialty providers to deliver comprehensive hepatology care.
Open to both new graduate and experienced APPs. Experience in hepatology, gastroenterology, or internal medicine preferred, but not required. Structured onboarding and orientation program available to support new graduates transitioning into practice. Key Responsibilities Provide high-quality, evidence-based care to patients in an outpatient hepatology setting. Perform comprehensive patient assessments, develop treatment plans, and manage chronic liver disease conditions. Collaborate with physicians, nurses, and specialty teams across multiple sites to coordinate patient care. Participate in patient education, care coordination, and follow-up management.
