The Registered Nurse will provide necessary home visits for patient assessment, treatment, support, and crisis management, including pronouncements of death and admissions. Responsibilities also include developing comprehensive service plans and revising nursing care plans in collaboration with the patient’s physician and the Interdisciplinary Care Team (IDG).
Candidates must be a graduate of an accredited nursing school and hold a current State Registered Nurse License, along with a valid driver's license and required auto insurance. Two years of RN experience in a healthcare or clinical setting is preferred.
Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing Current State Registered Nurse License Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance Two years of experience as an RN preferred in a healthcare/clinical setting

Provide home visits as needed for patient assessment, treatment, and support.
