Provide direct patient care under the direction of an RN, including assisting with safe transfers and ambulation. Maintain accurate documentation of patient care and communicate changes effectively to ensure continuity of care.
Must hold a current CNA license and have at least one year of experience in a hospice, nursing home, or home health setting. Requires a valid driver's license, reliable transportation, and current auto liability insurance.

Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/Homemaker Plan of Care. Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy Provide necessary skills to appropriately report/communicate changes and document pertinent information and care provided to patient to ensure continuity of care. Practice accepted infection control principles.
