The Case Manager supports young adults (ages 18-25) in securing and maintaining stable housing by conducting intake assessments, developing service plans, and connecting clients to essential resources like education, employment, and medical/mental health services. Responsibilities also include monitoring vital signs, assisting with basic medical procedures, and providing physical support like lifting.
Candidates need 1-2 years of case management experience working with populations experiencing homelessness, mental illness, or trauma, and must possess a valid Driver's License. A Bachelor's degree in a related field is preferred, along with knowledge of Housing First and trauma-informed approaches.
The Supportive Housing Case Manager - CNA (Ages 18–25) supports young adults in securing and keeping stable housing while building life skills and independence. Using a positive, youth-centered approach, you’ll carry a small caseload (12–18 clients), offer flexible one-on-one guidance, and connect them to education, jobs, and community resources. This is a rewarding role for anyone who enjoys cheering young people on as they step confidently into adulthood.
