$79,310 - $93,184

The PACE Clinic Registered Nurse is responsible for managing the nursing care essential to the assessment, promotion, maintenance and restoration of participant’s health and wellness within their scope of practice. Will work to maximize the participant’s health outcomes and participant experience by identifying participant needs, completing pre-visit planning, supporting the clinic visit and providing closure to the clinic visits, including scheduling follow up appointments, testing, lab work, and participant instructions. Will respond to walk ins and/or triage calls. Responsible for thoroughly and appropriately completing clinic visits designated as nursing visits.
Demonstrates a working knowledge of best practices in nursing and maintains professional boundaries.
Confers with team members for effective and timely resolution of participant needs.
Maintains flexibility in schedule and responds to unexpected emergencies and changes in daily assignment to meet organizational needs.
Serves in the on-call nurse rotation.
Establishes therapeutic relationships with participants and families from a variety of cultural, ethnic, and socioeconomic backgrounds.
Demonstrates interpersonal skills with all levels of medical, nursing, administrative, and other interdisciplinary team members to effectively and efficiently provide level of service required by participants.
Facilitates communication with community agencies and providers including pharmacies, hospital departments, and extended care facilities to optimize participant transitions and the provision of required care services.
Demonstrates exemplary telephone and virtual intrapersonal communication skills to help facilitate the development and nurturing of a caring relationship and experience for a diverse population.
Completes nursing responsibilities including but not limited to medication reconciliation, and timely completion of required electronic medical records.
Effectively evaluates & communicates care, needs, and provides teaching and instruction to meet participant’s care goals.
Provides participant/caregiver education and training specific to identified needs. Responsible for the assessment and treatment of the PACE population.
Screens, communicates, and responds to participant needs appropriately.
Triages and responds to participants’ telephone calls using approved protocols.
Accountable for direct and indirect quality care for the PACE population.
Acts as a content expert and clinic resource by providing staff and participant education
Assists in the evaluation of medication compliance and the effectiveness of prescribed medications.
Assists in the communication with pharmacy providers.
Attends and participates in participant/family meetings, and IDT meetings as requested.
Confers with the Interdisciplinary Team for Plan of Care adjustments.
Maintains confidentiality of all participant events, discussions, and records.\
Accurately documents all pertinent information regarding the client in the EMR.
Demonstrates compliance with organizational mission, vision and values.
Performs clinical duties within the nursing scope of practice adhering to all local, state and federal regulations.
Utilizes information management to support decision making to improve participant outcomes, improve health care documentation, assure participant safety and improve performance in participant care, treatment, and services.
Documents provision of care and services in the medical record.
Participates in departmental and agency quality improvement activities.
Follows established procedures, protocols, and standard precautions related to nursing services provided.
Properly cleans durable medical equipment as necessary.
Provides for continuity of services in home, community, or clinic through effective information management, both written and verbal communication.
Maintains clinical competency and performs duties within scope of practice.
Promotes interdisciplinary collaboration by prioritizing issues and directing communication to the appropriate team members.
Ensures compliance with regulations, professional licensing, and standards of practice.
