The role involves providing comprehensive patient care through assessment, diagnosis, and developing individualized treatment plans focused on rehabilitation and mobility enhancement in home health settings. Responsibilities also include educating patients and families, collaborating with interdisciplinary teams, and maintaining accurate documentation of patient progress.
Candidates must be a Licensed Physical Therapist in Washington state (or obtain licensure before the first day) and possess one to two years of experience as a Physical Therapist. A current BLS certification is also required for this position.
