The Wellness Coordinator proactively coordinates client healthcare to maintain high-quality health services, ensuring the integrity of health plans through consultation and training staff on assigned subjects to meet competency standards. Duties include participating in case conferences, assisting with required paperwork like reviews and plan of care development, and performing initial consumer admission assessments.
Candidates must possess an Associate of Science in Nursing with 1-2 years of preferred nursing experience, or equivalent work experience, along with a State of Indiana Licensed Practical Nurse Certificate and CPR certification. A minimum of six months of hands-on experience related to job duties is required, preferably working with persons with disabilities, and candidates must meet driver insurability requirements.
Education: Associate of Science in Nursing with 1-2 years of nursing experience preferred or equivalent of work experience required. State of Indiana Licensed Practical Nurse Certificate and CPR certification required. Core A & Core B Train-the-Trainer certification preferred, but not required. Work Experience: Minimum of six months of hands-on experience related to the job duties and responsibilities. Prefer individuals with experience in working with persons with disabilities. General Requirements: Must have a valid driver’s license, acceptable driving record, meet agency’s driver insurability requirements, reliable vehicle, and up to date automobile insurance. Training Requirements during Employment: Comply with all internal training requirements, as well as professional training that will ensure department compliance with regulations and best practices. Physical Requirements: Significant bending, lifting and stopping. Must be able to demonstrate the ability to lift 100 lbs. Considerable physical activity is required. Requires heavy physical work, lifting, pushing, or pulling. Work environment involves exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment. Repetitive physical activity such as data entry.
