The coordinator will develop and monitor individual treatment and support plans while leading the Person Centered Planning process, evaluating progress and safeguarding client interests. Responsibilities also include providing counseling, training caregivers, assisting in accessing community services, and managing healthcare provider selection and financial resources.
Candidates must possess a Bachelor’s degree in a human services field, adhering to Medicaid Provider Manual Guidelines, and hold a valid Michigan driver's license. The role seeks individuals excited to make a difference and who are dedicated to serving the population.
