This role ensures patients are prepared for transitions between care levels by reviewing medications for appropriateness, addressing discrepancies with prescribers, and providing comprehensive medication counseling to patients regarding their regimens and resources. The pharmacist may also participate in specialty consults like anticoagulation, heart failure, transplant education, and high-risk discharge counseling.
A Doctor of Pharmacy (PharmD) degree from an ACPE-accredited school is required, along with a Texas State Board of Pharmacy license and Basic Life Support certification upon hire. Preferred experience includes completion of a PGY-1 residency or two or more years of clinical pharmacist experience.

Required Education Doctor of Pharmacy (PharmD) degree or equivalent from an American College of Pharmaceutical Education (ACPE) accredited school of pharmacy required. Experience Good general health and stress-coping ability. Licenses and Certifications (PHARMD) DOCTOR OF PHARMACY Upon Hire Licensed Pharmacist by Texas State Board of Pharmacy (TSBP) Upon Hire (BLS) BASIC LIFE SUPPORT certification with a course accredited by the American Heart Association (AHA) Upon Hire Preferred Experience Completion of a PGY-1 Pharmacy Practice Residency from an ASHP-accredited residency program preferred. 2 years or more of clinical pharmacist experience
