$80,000 - $120,000 / YEAR
Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury, working as part of a home health team to meet the patient’s plan of care goals. Responsibilities include observing movements, conducting assessments, documenting progress, modifying the care plan, assisting with exercises, and educating patients and families on the care plan and self-care.
Candidates must be a graduate of an accredited Physical Therapist Program and hold an active PT license in Michigan, welcoming new graduates. A minimum of 1 year of clinical experience, preferably in home health within the last 5 years, along with CPR certification and strong clinical judgment, are required.
