The physician will provide primary care services to frail participants of the Neighborhood PACE program across multiple long-term care settings. This includes working in PACE clinics, at home, and in assisted living and nursing facilities.
Candidates should have significant experience as a practitioner for frail adults, with a preference for those certified in Internal Medicine or Family Medicine. Geriatrics certification is desirable but not required.
NeighborHealth is seeking a PACE Physician to provide comprehensive primary care to older adults with complex medical and functional needs enrolled in our nationally recognized Program of All-Inclusive Care for the Elderly (PACE). $229,200 to $271,200 based on experience. Physicians may also be eligible for federal and state loan repayment programs. What You'll Do: Monday through Friday schedule, 40 hours per week, 8 hours per day. Provide primary care services to frail participants enrolled in the PACE program Care delivered across clinic, home, assisted living, and nursing facilities Care in all settings is provided in concert with an interdisciplinary care teams of health professionals in a progressive health care organization.
