The Physical Therapist will conduct comprehensive evaluations and develop individualized treatment plans for patients in their homes. They will implement therapeutic techniques and collaborate with other healthcare professionals to ensure optimal patient outcomes.
Candidates must have a current and valid Physical Therapist license in Florida and a minimum of 6 months of experience in a relevant healthcare setting. Strong clinical skills and a patient-centered approach are essential.

Patient Assessment: Conduct comprehensive evaluations of patients' physical abilities, limitations, and functional needs to accurately diagnose impairments and develop individualized treatment plans. Therapeutic Interventions: Implement a variety of therapeutic techniques, including manual therapy, therapeutic exercises, and gait training to improve patients' mobility, strength, balance, and pain management. Functional Training: Assist patients in developing compensatory strategies, adaptive techniques, and assistive device training to enhance their independence in activities of daily living. Patient Education: Educate patients and their families on proper body mechanics, home exercise programs, and injury prevention strategies to promote long-term health and well-being.
Work collaboratively with physicians, nurses, occupational therapists, and other healthcare professionals to develop comprehensive care plans and ensure optimal patient outcomes.
Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries.
Adhere to all relevant regulatory standards and guidelines, including HIPAA and Medicare regulations.
Licensure: Current and valid Physical Therapist license in the state of Florida.
Minimum 6 months of experience in a hospital, skilled nursing facility, or rehab is requested.
Strong clinical skills, excellent communication and interpersonal abilities, and a patient-centered approach to care.
Strong work ethic, reliability, and a commitment to providing high-quality care.
Reliable transportation to visit patients in their homes.
