The Physician functions as a key member of the interdisciplinary team, developing comprehensive care plans, monitoring member health status, and providing geriatric and medical expertise. Responsibilities include providing preventive and episodic care, treating acute and chronic conditions, and supervising nurse practitioners or clinical nurse specialists.
Candidates must possess a Doctorate degree in Medicine and hold required licensure including DEA registration, NPI, and CPR & AED certification, along with a Wisconsin Driver's License. A minimum of one year of recent primary care experience with an at-risk or elderly population is required, and Board Certification in Internal Medicine or Family Practice is necessary, with advanced certification in Geriatrics preferred.
Physician, Full-Time, Day Shift
Community Care is hiring a Physician who enjoys team collaboration, community-based work, being part of a mission driven organization and who shares our mission to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This is a full-time opportunity based out of our Racine office. May be assigned on a short or long term basis to other Community Care PACE locations as business needs dictate.
Functions as a member of the interdisciplinary team and participates in the development of the comprehensive plan of care and ongoing monitoring of members health status, providing geriatric and medical knowledge when needed. Provides leadership to the team in utilizing appropriate resources to gather information and guide decisions. Assists the team in creative care planning for challenging cases. Works as supervising physician with the nurse practitioner or clinical nurse specialist at designated site. Provides preventive health care, episodic care and health promotion for members, and arranges referrals as indicated. Performs physical examinations, functional status evaluation and/or mental status evaluations as indicated. Treats acute and chronic conditions based on member history, clinical assessment, laboratory and diagnostic testing, and member preferences and goals of care. Obtains a complete health history and records findings, and maintains documentation of health care services. Exercises clinical judgment when evaluating members for disease process, orders appropriate and clinically indicated testing, reviews and/or interprets screenings and selected diagnostic tests. Discriminates between normal and abnormal findings associated with aging, pathological processes, lifestyles, and/or family relationships as influenced by chronic illness. Develops plan for follow up based on results, which may include consultation. Identifies and describes behavioral patterns of the developmentally disabled, physically disabled and aged chronically ill and integrates developmental processes, life style and family relationships. Triages status changes and facilitates appropriate management of problems. Treats and manages members' acute chronic illnesses. Regulates and adjusts medications as needed. Maintains documentation of provided, coordinated and contracted health care services. Educates members and families regarding health maintenance and acute and chronic illnesses.
