Physicians will lead within a collaborative Interdisciplinary Team (IDT) model, providing longitudinal primary medical care to PACE participants across multiple settings including home and virtually. Key duties involve driving clinical decision-making, coordinating whole-person care, and supporting participants through hospital and skilled nursing facility transitions.
Candidates must be a graduate of an accredited School of Medicine or Osteopathy with at least 2 years of post-residency experience, holding an active, unrestricted California MD/DO license and CPR certification. A strong preference is given to those Board Certified in Internal Medicine or Family Practice, with experience caring for frail and elderly populations being essential.
