The Nursing Care Coordinator will coordinate a patient care delivery system for a designated group of patients to achieve high-quality, cost-effective outcomes across the continuum of care, optimizing patient health status. This involves performing duties in ambulatory settings and assisting complex patients during care transitions across multiple settings while providing targeted interventions to prevent hospitalization and emergency room visits.
Required qualifications include a minimum of three years of Registered Nurse (RN) experience or equivalent Licensed Practical Nurse (LPN) experience, along with a Bachelor's degree in Nursing (BSN). Candidates must also possess effective communication skills, the ability to make independent clinical judgments, and current RN licensure in Nebraska or a compact state, along with Basic Life Support (BLS) certification.
