The RN Case Manager assumes primary responsibility for a patient caseload, involving assessment, planning, implementation, and evaluation of nursing care to maximize patient comfort and health. This role requires coordinating care by communicating with physicians and staff, maintaining documentation, and teaching caregivers.
Candidates must be a graduate of an accredited School of Nursing and hold a current Registered Nurse license in Georgia, along with at least one year of RN experience, though hospice experience is preferred. Essential requirements include CPR certification, excellent assessment and problem-solving skills, and a valid driver's license with an insured vehicle.

Ennoble Care is hiring for a Full-time Hospice Registered Nurse Case Manager to work within and around the Douglasville / Cobb County, GA region. As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician and the interdisciplinary group in providing skilled nursing assessments, planning, and care in order to maximize the comfort and health of both patients and their loved ones/caregivers.
Starting salary is $85,000 per year
