The Case Manager is responsible for providing documented individual and group clinical services and case management to clients with mental health, substance abuse, or dual diagnosis problems. They will develop, implement, and revise individual treatment plans for assigned clients and refer them to appropriate community services as necessary.
Candidates should have at least one year of experience in a substance abuse treatment or case management role. A high school diploma or equivalent is required, and LADC I or CADAC certification is preferred but not mandatory.
