The primary functions involve administering nursing care to patients of all ages in their residences and coordinating care with the Interdisciplinary Team, patients, families, and referring agencies on a PRN basis. The role also assumes responsibility for and participates in the coordination of care.
Candidates must be graduates of an accredited Diploma, Associate, or Baccalaureate School of Nursing and hold a current state Registered Nurse license, along with a valid driver's license. Two years of Hospice experience is preferred, and reliable transportation with valid auto liability insurance is mandatory.

Primary functions are to administer nursing care for patients of all ages in their place of residence and to coordinate care with the Interdisciplinary Group/Team, patients, families, and referring agency on a PRN basis. Assumes the responsibility for, and participates in, the coordination of care.
Education: Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Current state license as a Registered Nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact. Current driver’s license.
Two (2) years Hospice preferred.
Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Specialized skills preferred in palliation and end-of-life. Must read, write and comprehend English.
Reliable transportation and valid auto liability insurance.
