The Hospice Physical Therapist provides coordinated care to patients of all age groups, planning, implementing, and evaluating patient care plans. They conduct physical therapy treatment programs based on medical referrals and collaborate with the Interdisciplinary Group/Team.
Candidates must be graduates of an accredited Physical Therapist Education Program and hold a Doctor of Physical Therapy degree. A current state license to practice physical therapy and two years of experience in an acute care or rehabilitation setting are also required.
Description Description A registered professional who provides coordinated care to patients of all age groups. Plans, implements, and evaluates patient care plans to maintain patient well-being. Plans, organizes, and conducts physical therapy treatment program based on the medical referral and their evaluation. Collaborates with Interdisciplinary Group/Team (IDG/IDT)
Education: Graduate of a Physical Therapist Education Program that is approved by the Commission of Accreditation in Physical Therapy Education (CAPTE) or a successor organization of CAPTE. Doctor of Physical Therapy required. *Note: Exceptions may exist for education and licensure prior to 2009 and if educated outside of the United States. (42CFR 418.114(b)(7)(ii-vii).
Current state license as a physical therapist, or a multi-state license issued by a state member of the Physical Therapist Licensure Compact (PTLC) if practicing in a state member of the PTLC to practice physical Therapy.
Two years’ experience as a physical therapist in an acute care or rehabilitation setting recommended.
Therapy skills as defined as generally accepted standards of practice. Good interpersonal skills. Knowledge of durable medical equipment.
