The coordinator is responsible for ensuring patient assessments comply with established standards, which involves accurate data extraction from clinical documentation and timely submission for Medicare patients. Key duties include educating staff on proper documentation, serving as the primary resource for Quality Indicator coding and IRF-PAI completion, and ensuring accurate data entry and transmission to CMS/UDS.
Candidates must be a licensed or certified clinician in healthcare (e.g., RN, PT, SLP) or hold a QI Credential, and possess a minimum of one year of healthcare experience, along with the ability to perform assessments. Preferred qualifications include experience in rehab or post-acute care, proficient teaching, and strong communication skills, with a requirement to obtain UDS IRF PPS Certification within two years.
· License or Certification: o Licensed or certified clinician in healthcare (RN, LPN, PT, PTA, OT, COTA, SLP, Recreational Therapist, Respiratory Therapist, case manager/social worker) o QI Credentialed o Obtain UDS IRF PPS Certification after 2 years in the PASC role and before the 3rd-year anniversary. ·
o Ability to perform assessments. o 1 year of healthcare experience. o Attend all EHC IRF-PAI trainings. o Preferred: Experience in rehab or post-acute care, proficient teaching, and communication skills · Effective oral and written communication skills · Strong organizational and critical thinking abilities · Detail-oriented and capable of meeting deadlines independently · Commitment to maintaining high-quality standards in patient assessments.
