The Nurse Coordinator maintains communication with patients, families, staff, and providers to ensure seamless coordination of patient care across the continuum within the Orthopedic program. This role involves managing incoming referrals, coordinating initial patient contact, and compiling patient lists in collaboration with physicians and Advanced Practice Providers.
Candidates must possess a Bachelor's Degree in Nursing and hold an active Connecticut RN license. A minimum of three years of experience is required in adult orthopedic acute care inpatient services, ambulatory care, or critical care settings.

Under the direction of the Manager, Director of Orthopedic Services, and Physician Director, the Nurse Coordinator works collaboratively with physician leaders, physicians, and staff of the Orthopedic program to ensure goals and objectives are successfully met. The primary role of the Nurse Coordinator is to maintain communication with patients, families, staff, and providers in order to ensure coordination of patient care across the continuum.
