The Medical Scribe functions as a personal assistant to the physician, primarily by documenting patient visits accurately in the EHR, gathering necessary information, and transcribing diagnoses, symptoms, follow-up instructions, and orders dictated by the physician.
Required qualifications include a High School Diploma or GED and at least one year of previous related medical office or scribe experience. A Bachelor's degree or Scribe Certification (CMS or MSCE) are preferred qualifications.
Functions essentially as a personal assistant to the physician; performing documentation in the EHR, gathering information for the patient's visit, and partnering with the physician to deliver quality, efficient patient care. Responsible for documenting accurate and detailed information of patient visits. Provide the highest quality customer service to patients, adhere to practice protocols and processes, and effectively manage the volume of work without compromising quality.
Accompany physician into the patient examination room in order to transcribe a history and physical exam, and document accurately the physician’s encounter with the patient and others present. List all proper diagnoses and symptoms as well as follow up instructions and prescriptions as dictated by the physician. Transcribe patient orders including laboratory tests, radiology tests, medications, etc. Document any procedures performed by the physician. Enter all data into the EHR.
