The Youth Care Coordinator provides targeted support services using the Systems of Care (SOC) model to coordinate mental health and community services for children and families impacted by trauma. This role focuses on preventing out-of-home placements by encircling families with support, working with youth in their homes and communities while incorporating the entire family in treatment options.
A Bachelor's Degree is required, preferably in Social Science, along with a minimum of one year of experience working with juveniles in the mental health or related social service field. Candidates must be fluent in both English and Spanish and possess a valid driver's license to transport clients.
$55K annual salary
