• Provision: Provides therapeutic clinical services to
• ACT program clients evidenced by: Conducting comprehensive assessment of psychiatric history, mental status, and diagnosis; physical health and dental health, use of drugs or alcohol; education and employment; social development and functioning; activities of daily living and family structure and relationships. Providing direct clinical services including individual supportive therapy and psychotherapy to Participants on an individual, group, and family. Assisting individuals in accessing resources and education related to basic needs such as benefits, food/nutritional support, financial support/education, transportation, and stable housing. Demonstrating the ability to recognize and assess suicide potential and risk and provide appropriate responses to the situation. Supporting individuals served and their families in crisis situations. Provides on-call crisis intervention as scheduled. Plans, structures, and prompts social and leisure-time activities on evenings, weekends, and holidays as assigned.
