The Nurse Practitioner will conduct in-home wellness risk adjustment assessments for Medicare and other at-risk members, focusing on evaluating overall health for accurate risk adjustment coding and value-based care. Responsibilities also include performing comprehensive health evaluations, various screenings, patient education, and coordinating care findings with primary care providers.
Candidates must possess a valid, unencumbered Nurse Practitioner license for Oklahoma and have access to reliable transportation for daily travel within a 55-mile radius. Preferred qualifications include previous in-home risk assessment experience and at least three years of patient care experience in primary care, adult, or geriatric settings.
