The Hospice Registered Nurse (RN) Case Manager oversees and coordinates the plan of care for a dedicated caseload of patients, managing comfort measures to ensure quality of life based on patient goals. This role involves completing home and facility visits, coordinating care among the hospice team in a designated territory, and maintaining accurate documentation per CMS guidelines.
Candidates must possess an active New Hampshire RN license and have a minimum of 1-2 years of direct care experience, with End of Life experience being preferred. Essential requirements also include excellent communication skills, reliable transportation, a valid driver's license, automobile insurance, a recent physical exam, and a two-step TB test.
