The Palliative Care Nurse Practitioner performs palliative care consultations and follow-up visits in community settings to manage symptoms and discuss goals of care. They coordinate with the patient's healthcare team and provide education regarding the trajectory of advanced illness.
Candidates must hold a current nursing license and a Master's degree from an accredited Nurse Practitioner program. A minimum of one year of experience as a Nurse Practitioner and one year of hospice or palliative care experience is preferred.
