The Physical Therapist will evaluate patients' functional limitations in home health settings and develop individualized treatment plans focusing on restoring function, improving mobility, and alleviating pain. Responsibilities also include providing therapeutic interventions, educating patients and caregivers, documenting progress, and collaborating with the interdisciplinary team.
Candidates must possess an active Physical Therapist license in Texas and professional liability insurance specific to Texas Physical Therapy. A graduate degree or higher in Physical Therapy is required, with experience in home health or rehabilitative settings being preferred.
