Evaluate patients' physical abilities and functional limitations in a home health setting, developing and implementing individualized treatment plans to restore function, improve mobility, and alleviate pain through various therapeutic interventions. This includes educating patients and caregivers, monitoring progress, documenting care, and collaborating with an interdisciplinary team.
Requires an active Physical Therapist license in Texas and professional liability insurance. A graduate degree or higher in Physical Therapy from an accredited program is necessary, with experience in home health or rehabilitative settings being preferred.

Essential Job Functions: Evaluate patients’ physical abilities and functional limitations in a home health setting Develop and implement individualized treatment plans to restore function, improve mobility, and alleviate pain Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies Monitor patient progress and adjust plans as necessary to achieve optimal outcomes Accurately document evaluations, treatment sessions, and progress notes using digital platforms Collaborate with interdisciplinary team members including physicians, nurses, and occupational therapists Conduct home safety assessments and make recommendations to prevent falls or re-injury Deliver compassionate care while empowering patients to achieve greater independence Active Physical Therapist license in the state of Texas (TX) Professional liability insurance coverage specific to your TX Physical Therapy license Graduate degree or higher in Physical Therapy from an accredited program Experience in home health or rehabilitative settings is preferred Strong ability to adapt care approaches to suit a variety of patient personalities and needs Up-to-date knowledge of evidence-based treatment techniques and best practices Excellent communication skills and a friendly, professional demeanor when educating and interacting with patients Multiple major medical plans (Medical, Dental & Vision) Spousal insurance options 401(k) plans available Paid Time Off (PTO) Internal awards and recognition programs Supportive team environment with flexible scheduling options
