The Social Worker's primary function is to assess the psychosocial symptoms of the patient and family within the core interdisciplinary group. This involves developing psychosocial outcomes and providing necessary psychoeducation, supportive counseling, and casework interventions to meet those outcomes.
The role requires functioning as an expert in the field, committed to professionalism, collaboration, and continuous growth within a hospice mission setting. The individual must be capable of performing psychosocial assessments and providing direct supportive services to patients and families.
