The role also involves assisting with administrative tasks, preparing and facilitating educational groups, coordinating care with providers, and maintaining compliant documentation.
Candidates must possess a High School Diploma or GED, and while two years of experience with a relevant population is preferred, a valid driver's license with a good driving record is required. The position is per-diem and located at the New Life Center in Putnam, CT.
