The Clinical Nurse - Navigator coordinates the care needs of assigned patients, developing comprehensive plans to manage care delivery across the patient care continuum. This involves partnering with patients and physicians to create customized care plans and collaborating with various healthcare professionals to ensure timely tests and treatments.
Candidates must possess an Associates of Nursing (ASN) and an active Registered Nurse (RN) license in Indiana or an NLC RN license, along with BLS certification. Three to five years of clinical experience is preferred, with at least three years of cardiac nursing experience being strongly preferred.
